- The assessed fee may increase from what was originally scheduled depending on what procedures are needed and performed.
- The assessed fee may also decrease from what was originally scheduled if the appointment is less complex than anticipated.
- We accept cash, checks, all major credit cards, Flexible Spending accounts (FSAs) and Health spending accounts (HSAs) for non-cosmetic services.
- Payment is due in full on the day and time of appointment.

Helpful Hints To Schedule Your Visit:
Type 1 Visit: For ESTABLISHED PATIENTS ONLY to follow-up on stable conditions, obtain medication refills, or for monitoring or retreatment of a few lesions.
Type 2 Visit: For NEW PATIENTS with 1-2 concerns, or needing a partial skin check. For ESTABLISHED PATIENTS with a new concern, follow-up on multiple issues, or needing lesion removal. This appointment type IS NOT appropriate for a full skin examination/full body mole check.
Type 3 Visit: For NEW or ESTABLISHED PATIENTS desiring a FULL BODY SKIN CHECK/FULL MOLE CHECK, assessment of a full body rash, more than 2 new concerns, initial hair loss visit, follow-up on multiple concerns, or anyone desiring additional time with Dr. Crute.
Surgical Visit: This visit will only be scheduled after an initial office visit assessment and discussion of necessary procedure.
Procedures: All procedure fees are in addition to the office visit fee. All procedures will be discussed and agreed upon prior to completion. Pathology fees (when necessary) are not included with the above listed fees.
Other Fees:
Occasionally, there may be additional fees for staff time, administrative work, or extra tasks that are done on your behalf. We will inform you beforehand if extra fees are involved. These include but are not limited to: pathology fees (may also be submitted through insurance if desired), forms such as FMLA or disability, prior authorizations, medical coding/billing forms for reimbursement, and administration of certain medications. Specific details regarding these fees is forthcoming.
Good Faith Estimate
-Effective January 1, 2022, you have the right to receive a “Good Faith Estimate” for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
-Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
-If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

Spectrum Dermatology Services and Offerings

General Dermatology
Adult and Pediatric conditions we treat include: acne, rosacea, eczema/dermatitis, psoriasis, warts, seborrheic keratoses, actinic keratoses, basal cell and squamous cell carcinomas, moles, melanoma, and skin aging.

Surgical Dermatology
Dr. Crute performs some basic dermatologic surgical techniques to treat various conditions such as abnormal moles, irritated skin lesions, and skin cancers based on condition, size and location.

Cosmetic Dermatology
All Cosmetic services rendered at Spectrum Dermatology are performed by Dr. Crute. Current cosmetic service offerings include SkinPen® microneedling, neuromodulator injections (Botox®, Dysport®, & Xeomin®), and chemical peels as well as skin product recommendations.

Skin Care Products
We carry Skinbetter Science® products which are exceptional quality, medical grade products only available through authorized physician partners. These products protect, repair, and enhance your natural skin and improve cosmetic outcomes.
