Our services are priced by appointment time, number of concerns, and complexity

Virtual Visit: Follow-Up Appointment – $100

For established patients to follow up on 1 concern that has already been addressed or to request medication refills

Virtual Visit: New Patient or New Concern Appointment – $150

For any new patient with 1 concern or an established patient with 1 new concern that has not yet been addressed

 

For an appointment to address more than 1 concern, please reach to us out so you can be scheduled appropriately
Pricing will reflect the number of concerns being addressed and will be communicated prior to  booking the appointment

  • The assessed fee may increase from what was originally scheduled depending on the number of concerns being addressed or appointment length.
  • The assessed fee may also decrease from what was originally scheduled if the appointment is less complex than anticipated.
  • We accept all major credit cards, Flexible Spending accounts (FSAs) and Health spending accounts (HSAs).
  • Payment is due in full on the day and time of appointment or at the time of appointment booking if scheduled online.
  • Prices are subject to change without notice.

Other Fees:

Occasionally, there may be additional fees for staff time, administrative work, or extra tasks that are done on your behalf. We will inform you beforehand if extra fees are involved. These include but are not limited to: pathology fees (may also be submitted through insurance if desired), forms such as FMLA or disability, prior authorizations, medical coding/billing forms for reimbursement, and administration of certain medications. Specific details regarding these fees is forthcoming.

Good Faith Estimate

-Effective January 1, 2022, you have the right to receive a “Good Faith Estimate” for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

-Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

-If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

-For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call the No Surprises Help Desk at 1-800-985-3059.
